Outreach services.

In addition to our Support Living services, we also provide Outreach services. This is a great opportunity for those living at home with their families, to access their local communities with 3 Trees staff.

Our outreach services

In addition to supporting individuals to live more independently within their own home, we also provide Outreach services designed to support people to network and fully integrate into the communities that they live in by promoting healthy social interactions,  life skills, and other interests. 

This is an ancillary service to 3 Trees’ main model of care (Supported Living).

Outreach is a model of care typically associated with supporting people who live in their family home to access their local communities during the day.

Outreach is the model of care that 3 Trees began their journey with back in 1999. Outreach has a very special place in our hearts and has provided us with many treasured memories throughout the years.

Outreach is not currently our primary model of care; however, we enjoy delivering these services and it can be a pathway into Supported Living within 3 Trees.  

Our Outreach services typically support individuals with:

  • Networking with peers,
  • Developing and maintaining life skills, 
  • Accessing college, or voluntary/paid work,
  • Using public transport,
  • Social and leisure activities such as attending clubs, gyms etc.,
  • Organising trips and holidays,
  • Personal care.
